Position Overview:

The Advanced Practice Nurse (APN) is responsible for working in collaboration with the Patient, PCP, and Medical Director and other palliative care team members in an active practice to deliver palliative care and related health care services to patients and families.  The APN will travel throughout the Chicagoland suburbs as far west as DeKalb. 

Experience and Skills

Job Requirements:

  • Master’s degree in Advance Practice Nursing
  • Training in hospice and palliative care and pain and symptom management
  • Licensed Advanced Practice Nurse in the state of Illinois
  • Valid Driver’s License
  • Valid Auto Insurance
  • 1 to 3 years of experience as an advanced practice nurse in the palliative healthcare setting
  • Experience working with patients of all ages
  • Communication skills, verbal and written
  • Organization skills
  • Computer skills
  • Competent in pain and symptom management
  • Able to use medical equipment
  • Ability to travel throughout suburbs as far west as DeKalb

Preferred Experience, Skills, and Qualifications:

  • Certified Hospice and Palliative Nurse
  • Certification by approved agency
