Position Overview

The Transitional Care Navigator is a resource registered nurse to patients, family members and staff of hospitals, physicians, and nursing homes. They are responsible for sustaining and increasing referral sources from the hospitals and nursing homes and other community sources. The Transitional Care Navigator uses their knowledge in pain and symptom management and the end-of-life to educate these referral sources about hospice and home health. This RN performs as a case manager for patients in the hospital and conducts initial goals of care meetings with patients and families to inform them about hospice services.

This RN will Float in the Chicago Medical District area 12PM to 8:30PM Monday thru Friday and alt weekends.  

Experience and Skills

Job requirements

  • Associates degree in nursing
  • Training in counseling
  • Registered Nurse licensed in the state of Illinois

Preferred Experience, Skills and Qualifications

  • Certified Hospice and Palliative nurse, preferred
  • 5 to 7 years of experience as a registered nurse in healthcare, preferably hospice
  • Experience in the hospital setting preferred
  • Experience in Hospice and Palliative Care desirable
  • Marketing experience desirable
  • Weekend rotation required
  • Communication skills, verbal and written
  • Conflict resolution skills
  • Multi-tasking skills
  • Creative thinking and problem-solving skills
