Job Description:

The Referral Coordinator / Admissions is the first contact with families, doctors and healthcare facilities inquiring about the agency. The Referral Coordinator is responsible for obtaining and securing an appointment for any possible inquiries relating to patient referrals and coordinating those functions necessary to bring the referral to conclusion. This position will be located in our Barrington, Illinois office. The Hours are 8:00am to 4:30pm

  • Answers intake phone calls and provides information regarding the agency and collects, processes and records information regarding patient referrals and admissions per current agency philosophy and as guided by management.
  • Communicates with physicians to notify of patients/families requests for an Explanation of Services patient and may be required to obtain patient’s medical history, if needed. Document patient information in EMR system per r current procedures and guidance from management.
  • Maintains communication with referral sources and other interested parties , such as facility staff, physicians, and family members, as appropriate, to keep all concerned up-to-date in regard to the progression of the referral, per current agency procedures and guidance from management
  • Maintains communication with agency/facility staff regarding appointments, updates to clinical status, and other information to help ensure successful encounters between agency/facility staff and patient/families persons they are scheduled to meet per current procedures and guidance from management. May be required to obtain and record patient reimbursement sources, and communicates such to finance department staff per current agency procedures and as guided by management.
  • Coordinates visits for prospective patients and/or others who are seeking information and/or the initiation of agency services with appropriate agency staff per current agency procedures and guidance from management. Immediately contact management as soon as scheduling issues arrive.
  • Tracks pending patient referrals, ensuring all requirements for admission are accomplished (including but not limited to: physician orders, consent for care, contracts, Medicare bed status, home health involvement, consent for care) until patients are admitted to agency services or until advised by management to close the referral without admission.
  • Completes on a daily basis all follow up relating to pending patients. Insures next steps are documented, phone calls to referral sources are documented and follow up appointments are scheduled.
  • Insures all consents for patients are obtained daily, correctly filled out, logged on the consent log and verified. If consents are missing notify Director immediately.
  • Maintain various logs, records and files per current procedures and guidance from management.


Experience and Skills:

  • High School diploma/GED required; undergraduate degree preferred
  • Two (2) to five (5) years of experience working in the healthcare field preferred
  • Strong customer service experience in a healthcare environment managing a large volume of inbound calls preferred and a proven track record in a fast paced, multi-tasking environment with strong prioritization skills needed
  • Understanding of hospice and palliative care: coordination of admission process
  • Must have a strong knowledge of medical terminology
  • Computer skills (Word, Excel, Patient Database, e-mail), voicemail
  • Communication skills both written and verbal
  • Organization skills
