Job Description:

Registered Nurse - Triage / Visit RN or LPN -  Barrington area - PM Shift - 4pm to 12:30am. This RN will Also work Every Other Weekend.

PLEASE NOTE! There is TRAVEL/DRIVING involved with this position.  There is Also TRAVEL/DRIVING to BARRINGTON to Answer Phones. 

The Triage / Visit Nurse for the 24/7 Team provides patients, families and caregivers assistance by answering questions, resolving concerns and facilitating smooth access to care through effective triaging of calls, as well as making visits to assess and manage symptoms. The Triage / Visit Nurse will coach and educate families, patients and caregivers regarding symptom management and end- of- life care to ensure comfort and quality of life. The Triage / Visit Nurse is responsible for reporting symptom calls and interventions to the Interdisciplinary Team, Nurse Case Manager, and primary physician/MD as appropriate.

  • Provides patient care visits for admission, assessment, change in location/level of care and pain and symptom management and support during the dying process
  • Triages incoming symptom calls from patients their families and caregivers
  • Coordinates and prioritize visits; dispatch appropriate staff,
  • Staffs Crisis Care cases when appropriate
  • Educates current and prospective patients and families/caregivers regarding hospice and the end of life to include oxygen safety, medication use and direct patient care as needed
  • Documents patient visits and phone calls in an accurate and timely manner
  • Completes orders of new medication, supplies, and DME for patients
  • Communicates with the patient’s physician and/or medical director regarding any changes in status
  • Collaborates with the interdisciplinary team effectively and appropriately
  • Assesses family and caregiver ability to provide safe care, carry out the plan of care and understand the proper functioning of all supplies and equipment
  • Enhances self-knowledge regarding new medical and hospice information
  • Communicates working knowledge of all services offered by the organization to patients, families and the community
  • Facilitates level of care changes when necessary, including transfer to an In-Patient Center (IPC)
  • Provides information, evaluate patients for eligibility within regulatory guidelines and complete consents correctly with patients and families and admit them to appropriate services as needed

Experience and Skills:

Experience and Skills

  • Associates degree in nursing, BSN preferred
  • Registered nurse licensed in the state of Illinois
  • 2-4 years experience as a registered nurse in the healthcare setting, preferably in hospice, critical care or emergencys.
  • Training in hospice and palliative care and pain and symptom management preferred
  • Valid driver’s license, auto insurance and reliable transportation required
