Looking for a high-energy RN with recent oncology, critical care or emergency room experience

JourneyCare, a leading healthcare organization serving the Chicago area, has an exciting opportunity for a RN for the RUSH and CHICAGO metropolitan area! The Hours are 11am to 7:30pm. Some weekends are involved.  May need to work a weekend every 3 to 4 weeks.  Also Must be able to drive to Hospitals such as Rush, UIC, Northwestern, just to name a few.

The RN Hospital Liaison will conduct initial Goals of Care meetings with patients and families in the inpatient hospital setting to assess eligibility, symptom management, and utilization of hospice benefits.  You will also have the opportunity to coordinate discharge planning to the home, skilled nursing facility or hospice in-patient setting with members of the interdisciplinary team, and serve as a resource to hospital physicians, case managers and social workers in regard to end-of-life care.  Your flexibility and adaptability to schedule changes, and willingness to travel to various hospitals within the Chicago metropolitan area are an integral part of this role.

The ideal candidate has a high energy, professional presence, is an excellent critical-thinker and multi-tasker, and is savvy with software programs, including Allscripts.  This RN should be comfortable working with patients and families and possess a strong critical care background, preferably with recent experience working in a hospital ICU, ER or oncology setting.  You will serve as a liaison in transitioning patients to hospice care and collaborate with physicians, referral partners, hospital staff, nursing facilities and other care providers. 

 

Experience and Skills:

  • Associates degree in nursing required
  • Training in counseling preferred
  • Registered nurse (RN) licensed in the state of Illinois required
  • Valid driver's license, auto insurance and reliable transportation required
  • Certified Hospice and Palliative nurse, desired
  • 5 to 7 years experience as a registered nurse in healthcare, preferably hospice
  • Experience in the hospital setting preferred
  • Experience in Hospice and Palliative Care desirable
  • Marketing experience desirable
  • Weekend rotation required
