The Licensed Practical Nurse (LPN) Chronic Care Coordinator will provide support for the Accountable Care Organization (ACO) and our Chronic Care Management program by coordinating a comprehensive plan of care. The LPN Chronic Care Coordinator will facilitate the transmission of information between providers and patients, as well as access to services needed. 


This will be a telephonic role where we will provide care coordination and triage support to independent physicians who provide treatment for chronically ill patients.  The LPN Chronic Care Coordinator will report to the Director of Innovative Programs and the position will be located in our Barrington, Illinois office.  

  • Provide advocacy, information and referral services to patients and families to address their medical and psycho-social needs
  • Communicate effectively with doctors and other healthcare professionals the status of client and their current needs
  • Review client's preference sheet and make referrals based on need
  • Advocate for and provide information to patients and their families based on the client's need
  • Establish relationship of trust and support that continues over time as needs of patients change
  • Represent JourneyCare in a caring and professional manner to doctors and other health professionals
  • Responsible to document each client interaction and fax information to provider, if required per program
  • Accountable to deliver exceptional customer service in accordance with JourneyCare’s mission, vision and values
  • Model the JourneyCare culture of excellent customer service with clients, caregivers, families, visitors, facility staff, and other team members
  • Knowledge of the hospice and palliative care industry including regulations and eligibility criterion
  • Motivated to enhance knowledge base regarding current and new medical and hospice information through in-service participation, continuing education, and self-study
  • Responsible to complete timely, accurate health assessment interviews
  • Additional responsibilities assigned by supervisor


Experience and Skills:

  • Licensed Practical Nurse (LPN) in the state of Illinois
  • Training in hospice, palliative care and pain and symptom management
  • Certified Hospice and Palliative Nurse preferred
  • Minimum of one year (1) working in a health care environment, prefer hospice and palliative care experience
  • Training in HIPAA, Infection Control, safety, personal care and documentation
  • Experience working with seniors and individuals of all ages
  • Dynamic, positive individual with outstanding customer service skills
  • Professional image and conduct with adherence to a business casual dress code
  • Excellent verbal, written, interpersonal and organization skills
  • Creative thinking and problem-solving skills
  • Ability to work independently with a core sense of job ownership and accountability
  • Organization skills
  • Computer skills (Word, Excel, Client Database, e-mail), voicemail
  • Marketing skills to promote JourneyCare and build cohesive relationships with health facilities
  • Ability to apply critical thinking skills and common sense understanding to carry out instructions furnished in written, oral, or diagram/map form
