Job Description:

The Team Care Coordinator for the LAPIS / QUARTZ Teams provides the first impression person for all the members of the Interdisciplinary Teams and responsible for performing diversified and confidential receptionist and administrative support duties for the IDG.  This position reports to the designated Team Leader and is responsible for coordinating and managing the overall non-clinical processes and systems in order to support the clinical team to provide excellent patient care. Supports communication between IDG team members, patients and families and all JourneyCare programs as appropriate.  This includes ensuring regulatory compliance as related to Medicare, State, Local, and The Joint Commission and JourneyCare regulations.

This position will be located in the Glenview, Illinois office.  

  • Ensures the department is operating efficiently from an administrative perspective
  • Answers intake phone calls and provides information regarding the agency and collects, processes and records information regarding patient referrals and admissions per current agency philosophy and as guided by management
  • Coordinates and supervises bereavement volunteers and their assignments
  • Updates marketing materials on an on-going basis; support groups
  • Creates and coordinates the preparation of reports, analyzes data, and identifies solutions
  • Screens incoming calls and inquires to ensure that they are handled in the most efficient manner
  • Prepares and edits correspondence, presentations and reports using appropriate software and tools
  • Assembles check requisitions, expense reports and other statements for signature
  • Coordinates logistics for internal and external meetings
  • Manages sensitive issues by exercising sound judgment and interpersonal skills
  • Maintains knowledge of and adheres to agency policies and procedures
  • Researches, prioritizes, follows up and provides recommendations regarding issues, concerns and process improvements
  • Maintains database information and prepares spreadsheets to support reporting and analysis
  • Delivers exceptional customer service support to internal and external customers
  • Maintains and manages filing system (physical and electronic) and storage of documentation
  • Supports department with typing, photocopying, faxing and mail distribution
  • Other duties may be assigned or required for the performance of this position

Experience and Skills:

  • High School Diploma
  • Three (3) plus years administrative experience
  • Computer (Word, Excel, Power Point, databases, e-mail), voicemail
  • Communication skills verbal and written
  • Organizational skills
  • Customer Service skills
