Job Description:

This PER DIEM Nurse Practitioner position is for Northwest Community Hospital  in Arlington Heights. The Hours are 8am to 5pm.

Seeking Experienced APN with APN Experience in Hospital settings And APN Experience in Hospice and/or Palliative Care.

 The PER DIEM Nurse Practitioner for Northwest Community Hospital is responsible for working in collaboration with the patient, primary care physician, medical director, and other palliative care team members in an active practice to deliver palliative care and related health care services to patients and families.

Flexible hours available - self-scheduling

This NP will also need DEA after employment & NPI/Medicare in order to bill

Performs bedside palliative care patient/family evaluation: assists with diagnosis and management of acute and chronic health conditions.

Assists in care coordination for palliative care referrals.

Prepares and maintains accurate patient records.

Charts and documents to support sound medical practice and reimbursement for services provided.

Works with other staff in developing the palliative care program.

Recommends medications, treatments, and durable medical equipment.

Consults with the Medical Director and palliative care physician(s) as needed: consults primary physician of services provided and collaborates with other physicians.

Participates in Interdisciplinary Group Meetings.

Provides coverage of on-call during the extended hours and weekends as needed.

Complies with applicable laws and regulations with respect to Collaborative Agreements.

Supports and creates marketing and community education resources in advancing the Palliative Care Program.

Provides training and continuing education to staff in collaboration with the education department.

Assists in development of clinical practice guideline/standards/policies/procedures in support of quality palliative care.

Other duties may be assigned or required for the performance of this position.


Experience and Skills:

Master’s degree in advance practice nursing

Training in hospice and palliative care and pain and symptom management

Certifications, Licenses & Registrations Required:

Licensed Advanced Practice Nurse in the state of Illinois.
Certified Hospice and Palliative Nurse preferred

Certification by approved agency.

Job Experience Required:

2-3 year’s experience as an advanced practice nurse in the palliative healthcare setting

Experience working with patients of all ages

Specialized Competencies (Skills, Abilities, and Behaviors) Required:

Nursing skills

Communication skills, verbal and written

Organization skills

Computer skills (Word, Excel, patient database, e-mail), voicemail

Competent in pain and symptom management

Able to use medical equipment
Physical/Mental/Visual Job Requirements:

Frequently works under tight deadlines and with interruptions

Frequently sitting and working at the computer

Frequently working with confidential information

May be required to lift up to 25-50 lbs frequently and move up to 200 lbs occasionally with the assistance of a gait belt or Hoyer lift.

Assistance in administering medications that is ordinarily self-administered, when indicated.
