Job Description:

Seeking Per Diem / Registry Social Workers for the Chicago, South Chicago, and South Suburban areas. (LCSW Required). We really need the Per Diem Social Workers to help out with the counselor On Call schedule which is 5p-midnight or midnight-8am



The Per Diem Social Worker provides assessment of the needs of Journeycare patients and families.They also provide the patients and families with emotional and resources support.The social worker provides counseling regarding the dying process.


This PRN Social Worker may also support the Pediatric Team.   


~Assesses the social, financial, physical, resource and emotional needs of patients and families.

~Supports and educates patients and families regarding end-of-life issues and advanced care planning.

~Documents patient/family visits and phone calls and develops and updates individual care plans for patients/family.

~Attends and participates in interdisciplinary team meetings for patient/family care planning.

~Assesses bereavement issues and provides education and support for grief work.

~Identifies concurrent crises and makes appropriate referrals.

~Participates in patient chart audits.

~Provides internal and external education programs on Hospice and end of life social service topics.

~Attends on-site care plan conferences for patients we serve at facilities.

~Uses social work lead as a clinical resource and for assistance in caseload management

~Other duties may be assigned or required for the performance of this position.

~The position may supports pediatric patients.

Experience and Skills:

  • Masters in Social Work Required
  • LCSW  in Illinois Required
  • Training in patient assessment and counseling
  • Training in crisis management
  • Maintains professional licensure requirements
  • 2 or more years of experience in Social Work, medical preferred
  • Experience working with death and dying
  • 100% Travel Daily
  • Reliable vehicle with liability insurance
  • Valid Illinois Drivers' License


Bilingual in Spanish (Speak, Read, and Write) a plus
